Responsibilities |
The Diabetes Educator works closely and with the direction of the providers in the Endocrinology department. Performs professional nursing, consultation and educational services for persons with diabetes. Responsible for educating patients on lifestyle changes involved in diabetes diagnosis including nutrition, medication, physical activity and glucose testing to include education on diabetic pumps and sensors. Collects patient data to monitor patient health status and proactively manages the patient’s condition.

Required Qualifications |
Candidates must be certified as a Diabetic Educator (CDE) and have current and valid professional license, and/or certification, and/or degree required for their discipline. Additionally, candidates should have two years of experience caring for individuals with diabetes.
The successful candidate must be highly organized, service oriented, and possess creative problem solving skills. Excellent communication and interpersonal skills are necessary. The ability to work independently and as a team is essential. Maturity, flexibility, and an ability to remain calm and poised under difficult situations are vital. Must be able to maintain confidentiality as appropriate. Must possess the ability to obtain and apply new technologies as needed.

Knowledge/Skills/Abilities |
- Professional knowledge and skill as a diabetes and health educator with in depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns.
- Ability to investigate and analyze information and draw medically appropriate conclusions.
- Ability to design, develop, deliver and evaluate educational programs and materials.
- Ability to work with diverse population and represent organization in a positive manner.
- Knowledge of HIPPA, regulatory requirements, including state, federal and local regulations.
- Technical and clinical knowledge of diabetic medications, sensors and pumps and possess the ability to train and give guidance on the use of pumps and sensors and medication sensors.
